Full Membership Pack

New Applicants Only: Looking to join the Glass City Dog Park?  If you are unable to complete the online application (which is preferred and much faster to get your membership approved), this document packet contains all you need to do so. NOTE: If you complete the paper application, you MUST pay by mailing a check to the address listed in the packet. We are unable to accept debit/credit/PayPal over the phone or through the mail. 

Medical History Form for Existing/Renewing Members

Current/renewing members can use this form during the year to update their veterinary records. NOTE: You do not have to use this form. Instead, you can email (or have your vet email) just the updated vaccine record/due dates. Please email membership@glasscitydogpark.org.
